Home and Community Based Services Program SUPPORT Services for eligible individuals of all ages with serious or chronic illness or disabilities who want to remain in their own homes.
Some services that QCMI coordinates based on client’s needs are:

The Home and Community Based Services Program Medicaid through HMO services eligible adults who need 24 hours supervision. Individuals may be served in an Assisted Living Facility or in a Community Care Foster Family Home that provides personal care, homemaking services, and transportation as needed. Individuals may receive home health services, respite care and/or adult day health services as part of their services plan through an HMO. To receive services, a person must be eligible for Medicaid, require nursing facility level of care, and meet other program related requirements, such as enrolled to and approved by chosen HMO.
